How to register

We are happy to register you as a patient at Beech House Surgery if you live within our practice area.  Please speak to a member of reception to establish if your address falls within our boundary.  However, as an general overview, and dependent on your actual postcode, our boundary is  this side of the A55, as follows:

  • From St Asaph along to Caerwys roundabout – Junction 31 (McDonalds)
  • Down to Babell and Nannerch
  • From Nannerch down to Gellifor, Rhewl and Bontuchel
  • Then across to Cyffylliog and then up to, but not including Llansannan
  • From Llansannan, it then goes up to Bodelwyddan, this side of the A55 only, and back across to St Asaph
  • St Asaph boundary finishes this side of the A55.

You will be asked to complete a Patient Registration form and a New Patient Health Questionnaire and you will also be invited to make an appointment with the Practice Nurse for a  health check.

You will be registered under under a named GP Partner.   If you have a preference of your named GP please speak to reception team and they will add this to your records. However, the GPs operate a shared list, and you can make appointments to see any of our doctors, not just your named GP.

Please do not hesitate to advise if you need any assistance in completing the forms.

You can only be a registered patient with one GP surgery so if you register with us you will be de-registered with your current GP surgery and your previous medical notes will be sent to us.

Unfortunately, we cannot take any requests to register if your address does not fall within our practice area.
