If you change your name, address or telephone number, please complete a Change of Address form (this link opens a new page). Please return the completed form via email to enquiries.w91033@wales.nhs.uk, in person to either the Denbigh surgery or St Asaph surgery, or by post.
If you move outside the Practice area, you may need to find a doctor local to you. There are instructions on what to do when you move to a new area on the back of your NHS medical card.
